1. Start speaking/writing/reading/thinking in English. 

   - Get a subscription for some English newspaper (preferably The Hindu) and start reading it for atleast 30 minutes a day.

-Take up any article that fascinates you and write a summary of that article in a diary. With time, you will be able to write good articles with a proficiency in reading English.

-Start talking to yourself in English. Look at yourself in mirror and make conversations with yourself in English. You will start enjoying this language.

2. Join certain group of people who are keen to learn English. You will grow with them at a good speed. 

3. Start watching English movies with Subtitles.

4. Maintain a diary of every new word with its meaning that you come across in your day.

5. For Grammar, you have only 1 way. Start practising books that teach you basic grammar.

6. For Vocabulary, there is a book "Word Power Made Easy". It is the best book to start of with a great vocab session everyday for a month and with an increase in almost 20 words a day in your current vocab status.
